Member Rules & Expectations

A quick note before the specifics: DPC works because it's a real partnership. Here's what we ask of every member, and what you can expect from us — summarized in plain language from your Patient Agreement.

Membership & Billing

  • Term & renewal: Membership runs on a 12-month term and automatically renews each year unless either you or the Practice give 30 days' written notice to cancel.
  • Monthly dues: Billed automatically by credit/debit card or bank draft on the 1st business day of each month.
  • Enrollment fee: A one-time, non-refundable enrollment fee is due when you join, in addition to your first (prorated) month's dues.
  • Cancelling: You may cancel anytime with 30 days' written notice. If you cancel early, any unused portion of that month's fee is refunded on a per-diem basis.
  • Re-enrollment: If your membership lapses and you'd like to rejoin later, re-enrollment is subject to space availability and a $200 re-enrollment fee.
  • Fee changes: If dues or included services ever need to change, we'll give 30 days' written notice. If you don't agree to the change, you can cancel before your next payment is due.

What This Membership Is (and Isn't)

  • Not insurance: This membership doesn't replace health insurance. It does not cover hospitalization, emergency room visits, or specialist fees — we recommend keeping a separate insurance plan (even a high-deductible one) for those.
  • Medicare: Dr. Daquino has opted out of Medicare. Neither you nor the Practice can bill Medicare for services provided here. If you're on Medicare or become eligible while a member, please let us know right away.
  • What's included: Office visits, chronic disease management, wellness exams, and minor in-office procedures are covered under your membership, when within the scope of the practice and the provider. Medications, labs, and imaging are billed separately unless we've said otherwise — see the Pricing page for details.

Communication & Emergencies

  • Email and text are great for non-urgent questions, but aren't guaranteed 100% secure and aren't appropriate for emergencies.
  • For any real emergency, call 911 or go to the nearest ER — don't wait on a callback, text, or email.
  • If Dr. Daquino is temporarily away (vacation, illness, etc.), we'll notify members in advance when we can. Urgent issues during an absence should go to urgent care; those charges aren't covered under your membership.

This page is a plain-language summary for quick reference. Your signed Patient Agreement is the official governing document — contact the office if you'd like a copy.

How the Office Works

  • Scheduling: Call, text, or use the patient portal to request same-day or next-day appointments.
  • Visit length: Appointments are a minimum of 30 minutes — no rushing.
  • Between visits: Message us directly by phone, text, or video for quick questions — no need to book a full visit for everything.
  • Prescriptions: Sent electronically to your pharmacy, or dispensed in-office at a discount when in stock.
  • Labs & imaging: Ordered through our discounted local partners — we'll walk you through pricing before you go.
  • After hours: For a true, life or limb threatening emergency, call 911 or go to the nearest ER. Use the office line for urgent (non-emergency) member concerns.

Tips & Tricks for Members

  • Save our number as a contact — texting works great for quick, non-urgent questions.
  • For anything more urgent, call instead of texting or emailing. Phone calls always get the fastest response — texts and emails may not be seen for a few hours. If we're busy, just leave a message and we'll get back to you as fast as we can.
  • Book your annual wellness exam early in the year so it's never a scramble.
  • Ask about cash pricing before using insurance for labs/imaging — our discounted rates are often cheaper.
  • Keep a running list of non-urgent questions for your next visit — 30 minutes is enough time to cover several things at once.
  • Traveling? Reach out — many issues can be handled by phone or video even when you're away. Dr. Daquino is licensed in many states, so we may be able to help no matter where you are.
